Thank you for choosing our office! We are committed to doing everything possible to provide you with excellent and advanced dental care, while making your visit to our office a comfortable experience. We understand that your first visit to the dentist can feel overwhelming and we try to accommodate all new patients with a comfortable and organized facility.
Your Initial Visit
Your initial appointment will consist of x-rays, an examination and review of your treatment options as needed. Usually, treatment can be done or started the same day as the consultation after we receive medical history and review treatment plan. Some treatment plans may require an evaluation and a second appointment.
Please assist us by providing the following information at the time of your consultation:
-Any x-rays taken by a previous dentist, if applicable.
-A list of medications you are presently taking.
-If you have dental insurance please bring in your card, or pertinent information.
-You may fill out our New Patient Forms online to save time at your initial visit, otherwise, we ask that you come 10 minutes prior to fill them out in office.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e.,artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, antibiotics, etc.).
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
Financial and Insurance Information
Our patient care coordinators Tami, Bridjett, and Shayla are here to assist our patients with their scheduling, insurance, and account needs. We want to make it as easy as possible for you to receive dental care when you need it. Each day brings new interactions and we love getting to know our patients! Our team is genuine and we truly care about our patients. We want everyone to feel comfortable and relaxed in our office.

-Cash, Personal Check, or Money Order

-Health Savings Accounts (HSA) and Flexible Spending Account (FSA)
-Care Credit Healthcare Credit Card
We offer a 5% courtesy discount, and 7.5% senior discount (60+) to patients who pay for their treatment with cash, check, or debit card the day of treatment.
Insurance:
To help patients maximize their coverage, we encourage all our patients to become familiar with their own coverage as each plan/policy varies. We are an in-network provider with Delta Dental Premier and we can help submit claims to numerous other dental insurance companies as well.
**We are currently not accepting any new patients with state insurance plans (state medicaid and state medicare).
